Where do I submit my licensing forms?
All licensing forms can be submitted to Effie Papadopoulos in the MedEd Office during or at the end of your 4th year. Please be aware that each state has differing rules and regulations regarding ACGME/AOA requirements. It is up to the student to research the application process and their requirements under state guidelines. Please note that all requests received will be submitted directly to the state medical board. Former students that are applying to states that require proof of affiliation with NYU Lutheran Medical Center, will need to contact their school to request this documentation.
All students that have completed or will be completing their 3rd & 4th year rotations, and are looking to have licensing forms for the California Board to be completed by our facility need to do the following:
- Email Effie Papadopoulos at firstname.lastname@example.org requesting a L6 licensing form
- In the email you must list your date of birth, the last four digits of your social security#, school, and the year(s) that you rotated through NYU Lutheran Medical Center
For all other state licensing forms:
- Retrieve the form from the State Medical Board Website that you wish to apply to
- Complete only the personal information on the top of the form using Adobe Acrobat
- Email the form to email@example.com
- Make sure you list your contact information, school name and rotations that were completed at NYU Lutheran Medical Center
- Provide mailing specifications for completed forms
What Holidays and Vacations are observed for students?
All requests for time off for holidays, personal, interview, and sick days are department specific. Your Clerkship Director for each rotation will explain the department policy during the department specific orientation. All medical students with the exception of those scheduled under Sub I’s will have the following holiday break from Lutheran Medical Center.

- Memorial Day
- Independence Day
- Labor Day
- Thanksgiving Day including that weekend
- Christmas Day
NYU Lutheran Medical Center does not offer housing for students or assist in the allocation of housing.

What is the Evaluation Process?
For Medicine and Surgery Core Rotations:
Medicine – Students must print out the evaluation forms from the website and give to their attending that they worked with Dr. Giaccio completes composite for all evaluations it receives on students.
Surgery – Students record up to 3 requests thorough MedEd office for later pick up. Evaluation requests may include residents preferably PGY2 and higher &/or attendings. Envelopes are picked up by student to be given to resident/attending (usually within 24 hours of recorded request). Dr. Kilaru/Surgery Preceptor will receive a copy of all evaluations to be completed on student behalf. Department of Surgery completes composites of evaluations receives to be forwarded to the school
SGU – begin evaluation request at week 9 of rotation
NYCOM – begin evaluation request at week 7 of rotation
For all other 3rd year core rotations:
Evaluation requests for Pediatrics, OB, Family Medicine and Psychiatry (NYCOM students only) are initiated during the Sign Out process. All forms can be downloaded or picked up through MedEd office.
What is the Sign-Out Process?
At the end of each rotation, students must complete and a return a sign-out form to the MedEd Office.
All pagers, and keys must be returned after each rotation.
If students are leaving the NYU Lutheran Medical Center site and not returning, a more detailed sign out process is necessary which includes returning all NYU Lutheran property (linens, IDs, library books, etc).
If I am scheduled for an NBME exam on the Friday (last day of my rotation), when can I sign-out with the MedEd office?
All students are required to Sign Out and return all NYU Lutheran Medical Center property at the end of their rotation. Students may Sign Out just prior to their protected study time or may return to the MedEd office immediately after their exam. Please note that students on 12-week rotations receive 2 days of protected study and students on 6-week schedule receive 1 day of protected study time.
Those students whose exams are scheduled on a Thursday are required to report to NYU LMC on Friday of that week to complete any unfinished paperwork/documentation for rotation end, including Sign Out through MedEd office.
Those students who are scheduled to begin another rotation with us the following Monday will need to complete “Sign Out/Sign In” prior to the Monday start date. Please note that Library and Linen clearance will not be necessary for students continuing onto their next rotation.
Can I receive my flu shot at Lutheran Medical Center?
Flu shots are administered through the hospital’s Occupational Health Department, for our in-house students only. Flu shots are administered during the hospital-wide campaign to all employees and medical students who present a valid NYU LMC ID.
